Below are claim forms for NVQ and VRQ level 3 for use with the 2006 standards/syllabus, as well as the Level 3 Diploma. Please ensure that the appropriate form is completed.

All accompanying final IV reports must clearly show that portfolios or NPLs are complete and be dated within the appropriate timescale.
As stated on the claim forms, all portfolios must be made available immediately for awarding body audit should they be required.
Centres are requested to retain all portfolios until certificates have been issued to prevent delays with this process.
